How to Get a Arizona Pharmacy Technician License

Below you will find a summary of how to apply for and receive a license to practice pharmacy in the state of Arizona.

Designations

Pharmacy Technician License– This designation requires the PTCB or ExCPT license prior to application

Pharmacy Technician Trainee License– This designation does NOT require previous pharmacy technician license. If you have had a trainee license in the past, you will only be eligible to reapply once. If you had a trainee license in the past that expired, you will not be eligible to reapply. For more information please see theTechnician Trainee Reapply Application Requirements.

Applicants must have the following for the Pharmacy Technician License:

  • Proof of legal residency of birth date

  • Must be 18 years or older and have a high school diploma or equivalent

  • Government issued photo ID

  • Must have a passing score on either the PTCB or the ExCPT

  • Copy of a valid Arizona fingerprint clearance card

  • Any court documentation if applicable

  • Any disciplinary orders if applicable

  • Any license denial orders if applicable

  • License application fee — $82

The Process

Once you have all of the necessary documentation you’ll be able to apply for the license! You can use this link to create an account and apply.  The actual application is relatively easy to fill out.  It requires the basic information you’d expect from a form; name, address, contact information, birth date, social security number etc.

The second page of the form is for regulatory questions, so questions involving your past disciplinary actions, and denials of licenses, and addiction questions. If you answer yes to any of the questions, you will be prompted to answer questions regarding the questions and your past.

The next step requires your statement of US Citizenship and to indicate your place of birth.

The fourth page (step 5) is where you upload all of your documents. Proof of legal residency, photo ID, proof of the PTCB or ExCPT, documentation of any name changes, etc.

The 6thstep in the process is to check over all of your information and make sure it’s correct. After this you will be prompted to affirm that everything is correct, you didn’t lie, provide an E signature and submit your application. Once you have submitted your application you cannot make any changes

The Process – Pharmacy Technician Trainee License

The process for to apply for the trainee license is the same as for the full license, you just don’t need to provide proof of passing the PTCB or ExCPT.

Payment and Fees

The application fee for the full license is $82 and the fee for the trainee license is $60. Both fees are nonrefundable and can be paid by check or money order. If you wish to pay with a debit or credit card you must apply online.

Additional Information

Please be wary that only completed applications will be accepted by the Arizona Board of Pharmacy. This includes having all required documentation and filling out all the forms correctly and accurately.
