Job Type:

Full-Time | Monday-Friday, 8:00 AM - 5:00 PM
Travel Required: 50-75% for face-to-face visits within Miami-Dade County


Position Overview:

We are seeking a Bilingual Case Management Coordinator to join our dynamic and mission-driven healthcare team. This role involves coordinating healthcare services, conducting member assessments, and supporting overall member wellness through case management practices. The ideal candidate is organized, tech-savvy, compassionate, and fluent in both English and Spanish.

You will manage a diverse caseload, including members with supportive and medically complex needs, and will collaborate closely with healthcare providers, community resources, and the internal care team to promote positive health outcomes.


Key Responsibilities:

  • Conduct telephonic and face-to-face assessments to evaluate members' medical and social needs

  • Develop, implement, and monitor individualized care plans

  • Coordinate services including prior authorizations, PCP referrals, condition management, medication reviews, and community support

  • Engage members using motivational interviewing to encourage behavior change and health engagement

  • Document member interactions and care coordination activities in accordance with internal policies and regulatory standards

  • Collaborate with a multidisciplinary team to ensure quality, effective care delivery

  • Escalate any quality-of-care concerns through proper channels

  • Support members in navigating healthcare systems and making informed decisions


Required Qualifications:

  • Fluent in Spanish and English (bilingual required)

  • Minimum 1 year of case management experience

  • High school diploma or GED (verifiable)

  • Proficiency with Microsoft Office, including Word, Outlook, and Excel

  • Comfortable with field-based work and regular travel within Miami-Dade County

  • Strong interpersonal, communication, and organizational skills.
