Job Description:
As Case Management Coordinator, you will be responsible for Utilizing critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate health care outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.
NOTE: You will travel approximately 75% of the time within the region seeing members at home, in assisted living facilities and nursing homes.
Preferred Qualifications:
**A Must to have Case management experience required
**A nice to have a Long term care experience preferred
** Must be local- Candidates must reside in Titusville (Brevard County)
** Multilingual: Spanish/English/Rio (Preferred)
** Bilingual: Spanish/English a MUST
Education:
Bachelor's degree required - No Nurses. Social work degree or related field and MS Excel.
Schedule:
M-F; 8AM-5PM
