Position Summary
The Care Manager works in partnership with patients, families, caregivers and providers to deliver targeted care that is organized, coordinated, cost effective and patient-centered. The Care Manager collaborates with interdisciplinary staff and stakeholders within the care continuum to promote patients' optimal health through disease prevention and management strategies. Evidence-based interventions are applied to populations for cost efficient and outcome-based care. Outcomes include disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates. Must have discharge planning experience in an acute care setting by assessing and determining need for discharge planning and next level of care for Home Health Care, Skilled Nursing Facilities, Inpatient Rehab. or Long Term Acute Care Hospital discharges.

- 1.5 years of exp
- BLS
- Bachelors Degree Acute Care setting ranging from Critical Care to Med/Surg/Gen
BLS- Required
