Job position: Pharmacist (Inpatient)

Key Responsibilities
- Provide comprehensive clinical pharmacy services to inpatient units.
- Review, verify, and dispense medication orders in accordance with hospital and regulatory policies.
- Participate in patient care rounds and offer evidence-based clinical recommendations.
- Manage and monitor medication therapies, including antimicrobial stewardship, anticoagulation, pain management, and chronic disease management.
- Utilize EPIC for clinical documentation, order verification, and medication profile review.
- Oversee and troubleshoot Pyxis automated dispensing cabinets.
- Conduct medication reconciliation and patient counseling as needed.
- Support quality improvement initiatives involving medication safety and clinical protocols.
- Precept pharmacy students/residents if applicable.
Qualifications
- Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy required.
- Active and unrestricted state pharmacist license.
- Hospital or inpatient pharmacy experience required.
- Proficiency with EPIC electronic health record and Pyxis automated dispensing systems required.
- Strong knowledge of pharmacotherapy, medication safety, and clinical guidelines.
- Excellent communication, critical-thinking, and problem-solving skills.
- Preferred: PGY-1/PGY-2 residency, Board Certification (BCPS, BCIDP, etc.), experience in high-acuity settings (ICU, ED, oncology, etc.).
