We are seeking a full-time Licensed Nurses (RN or LPN) to participate in the case management and direct care of residents to assure excellence in the delivery of health services. You will be working in a retirement community setting in Lexington, MA.

Preferred: Experience in Geriatric population and Nursing home

Required: Can work with every other weekend and has an active license as an RN or LPN in Massachusetts

Day shift:

Monday - Friday

7AM to 3PM OR 8:30AM - 5PM
40 hours per week

We offer different pay for RNs and LPNs

Responsibilities:

Participate in daily plan of care meetings.
Participate in the development and execution of a written plan of care.
Make resident visits as per plan of care to observe and evaluate the resident's physical and emotional status.
Provide direct nursing care as necessary.
Obtain sputum, urine and other lab tests as ordered. Take TPRs, blood pressures, etc., as necessary. Participate in completing treatments as per physician order.
Ensure that all residents are treated fairly, and with kindness, dignity, and respect, ensuring person centered care.
Monitor medication oversight & treatment schedules.
Consult with the resident's physician in planning resident care, treatment, rehabilitation, etc., as necessary.
Assist in developing methods for coordinating nursing services with other services to ensure the continuity of the residents' total regimen of care.
Meet with residents, and/or family members, as necessary.
Respond to emergency calls to Residential apartments.

Qualifications:
Hold current nursing license in Commonwealth of Massachusetts (LPN or RN).
Experience working in a nursing setting with the elderly preferred.
Attention to detail and the ability to provide compassionate care required.
Must be able to show compassion, understanding, and patience when dealing with residents.
