Job Title: Licensed Practical Nurse (LPN/LVN)
Location: Various Locations in New York
Schedule: 3-4 Weekdays & Every Other Weekend


Position Summary:

We are seeking a Licensed Practical Nurse (LPN) to join our team and support clinical services in a unique retail-based setting. Under the direction of a Nurse Practitioner, the LPN will perform patient assessments, assist with point-of-care testing including COVID-19 screenings, handle documentation in the EPIC EMR system, and support day-to-day clinic operations.


Key Responsibilities:



1. Quality Patient Care

  • Obtain and document vital signs

  • Perform vision/hearing tests and CLIA-waived lab tests

  • Collect and process specimens for COVID-19 and other testing

  • Educate patients on treatment plans and next steps

  • Maintain a professional and courteous demeanor toward patients



2. Electronic Medical Record & Documentation

  • Accurately chart patient history, SOAP notes, and testing results in EPIC

  • Input insurance, demographic, and payment information

  • Ensure consent forms are properly completed and signed







3. Professionalism & Integrity

  • Maintain current licensure and certifications

  • Contribute to a positive, team-based clinic environment







4. Clinic Maintenance & Support

  • Order and manage supplies and weekly logs

  • Keep the clinic clean and organized

  • Report equipment or facility maintenance issues

  • Comply with universal precautions for safety and cleanliness







5. COVID-19 Responsibilities

  • Register patients, collect test samples, and notify results

  • Provide education based on testing outcomes

  • Maintain PPE standards and follow lab processing protocols

  • Support additional local locations as needed


Required Qualifications:

  • High school diploma or GED (verifiable)

  • Graduate of a State-approved Practical/Vocational Nursing program

  • Active, unrestricted LPN license in the U.S.

  • Minimum 1 year of relevant experience, including:

    • Use of Electronic Medical Records (preferably EPIC)

    • Taking vital signs and performing point-of-care testing
