We are seeking a Full Time Social Worker to join our Resident Health and Babson Assisted Care Departments. This individual will work closely with Residents and their family members to identify their psychosocial, mental, and emotional needs. You will be responsible for fostering an environment, policies and routines that enable Residents to maximize their individuality, independence and dignity and provide Residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

  • Assist in developing a comprehensive Social Services program that meets the needs of residents throughout the continuum of care.
  • Maintain focus on person centered care.
  • Serve as active contributor in care plan meetings, discharge planning meetings, family meetings and team meetings.
  • Ensure collaborative relationship with residents and families to make informed decisions.
  • Coordinate the provision of social services to residents to improve the quality of life for individual residents and respond to issues identified by residents and families to promote satisfaction with services.
  • Collaborate with and provide backup to Nursing Center Social Worker to ensure MDS and Care Plans are completed in accordance with regulations, resident rights are upheld, and progress towards Care Plan goals is made.
  • Meet with residents and families to assess personal wellness services
  • Provide assistance and coordinate services for residents who are coping with illness and loss and who are in need of therapeutic interventions.
  • Participate in interdisciplinary care meetings in Resident Health, Assisted Care, and Nursing Center.
  • Initiate, coordinate and provide support groups for residents.
  • Provide therapeutic interventions to assist residents in coping with transition and adjustment to Nursing Center or Assisted Care.
  • Make appropriate referrals to psychologist and psychiatrist as needed.
  • Complete Babson admission paperwork; give tours of Babson.
  • Assist residents with long term care insurance questions and Medicare A/Medicare D prescription drug plan questions.
  • Participate in reviewing and developing facility social work policies concerning resident care and quality of life to assure compliance with state and federal regulations.
  • Develop, maintain and utilize a listing of current community resources that are useful to residents and their families.
  • Understand and meet all state and federal requirements for social service documentation.
  • Document progress in meeting psychosocial needs of residents.
  • Work with interdisciplinary team to promote and protect Resident Rights and psychological well-being of residents.
  • Prevent and address resident abuse as mandated by professional licensure.
  • Provide annual staff training on Dementia, Resident Rights, Resident Abuse, Mistreatment, Neglect and Misappropriation of Property and Response to Complaints and Concerns.
  • Participate in developing written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues and appropriate Social Worker interventions.
  • Discuss advance directives with resident and family, including Health Care Proxy, Living Will, and end of life decisions such as feeding tube, do not hospitalize, comfort measures only, DNR, hospice, and plan for funeral home.
  • Provide support and education to residents and families to assist in understanding of placement and facility issues.
  • Participate in weekly Resident Care Committee to: review and anticipate problem areas of care, address catastrophic events, discuss transition of residents to different levels of care, coordinate discharge planning and make referrals for home care services.
  • Participate in New Resident Processing Committee and provide basic cognitive screening and recommend further evaluation as needed.
  • Maintain quarterly resident council for Assisted Care residents.
  • Maintain confidentiality of all pertinent resident information.



Qualifications:

  • Licensed Social Worker, LICSW
  • Experience with geriatric population preferred.
