Licensed Practical Nurse (LPN)
Loaction: Framingham, MA


Position Overview:
We are seeking a compassionate and motivated Licensed Practical Nurse (LPN) to provide medical support, care coordination, and rehabilitation services in a small, supportive setting. The LPN will deliver direct nursing care focused on health maintenance, medication management, client education, and care coordination as part of a multidisciplinary team.
Key Responsibilities:


  • Conduct health assessments, administer medications, and provide education on health maintenance.

  • Coordinate care to ensure follow-up services such as home health aides and physician appointments are in place.

  • Provide individualized and group health education sessions based on client needs (e.g., sleep hygiene, diabetes management, nutrition).

  • Assist with admissions and discharges from a physical health perspective, including medication reconciliation and follow-up care coordination.

  • Monitor clients' health status, document progress, and report any changes to appropriate team members promptly.

  • Advocate for clients and collaborate with healthcare providers to ensure comprehensive care.

  • Uphold safety, recovery, and treatment standards within the care environment.

  • Maintain accurate documentation in accordance with organizational policies and procedures.

  • Participate actively in team meetings, supervision, and trainings.

  • Maintain necessary certifications including CPR, First Aid, and others as required.


Minimum Qualifications:


  • Graduate of an approved nursing program with current LPN/LVN licensure.

  • At least one year of nursing experience, preferably with clients experiencing mental health challenges or chronic conditions.

  • Strong communication skills and ability to use good judgment.

  • Basic computer skills.

  • Valid driver's license and access to a reliable vehicle for client transport if needed.

  • Ability to work effectively in a team environment and serve diverse populations with respect and dignity.
