Day Habilitation Program - Licensed Practical Nurse (LPN)
Location: Ashland, MA
Position: Full-Time, First Shift
Schedule: Monday-Friday, 8:00 AM - 3:00 PM (flexible)
No weekends or major holidays



Overview:
The Day Habilitation Program serves adults ages 22 and over with developmental disabilities through therapeutic and recreational programming. We are seeking a Licensed Practical Nurse (LPN) to join the nursing team and provide daily nursing services including medication administration, G-tube feeding administration, first aid, and more. New graduates are welcome to apply.



Minimum Education Required:


  • High School Diploma or GED

  • Graduate of an approved nursing school with current LPN licensure in the Commonwealth of Massachusetts


Responsibilities:


  • Administer prescribed medications as necessary.

  • Consult daily with Team Leaders, Case Managers, Skills Instructors, Aides, and other relevant staff to share medical information.

  • Monitor and follow recommendations in accordance with the comprehensive written therapeutic service plan (DHSP/ISP) and interdisciplinary team.

  • Provide daily nursing notes and monthly summaries as needed. Maintain and update medical information in each client file.

  • Promote community inclusion and self-advocacy among program participants.

  • Collaborate with Team Leaders, Case Managers, and Skills Instructors to develop and implement DHSP/ISP curricula in relevant areas.

  • Provide emergency first aid and direct staff in medical crises.

  • Participate actively in staff meetings and in-service training as required.

  • Attend assigned trainings and maintain necessary certifications.


Qualifications:


  • Current LPN licensure in Massachusetts.

  • Minimum 1 year experience in a community healthcare setting preferred.

  • Strong oral and written communication skills with individuals, families, funding sources, consultants, community members, and program staff.

  • Ability to satisfactorily perform all essential duties.

  • Demonstrated competence in serving culturally diverse populations is strongly preferred.

  • Basic computer skills required.

  • Valid driver's license required.
